I have problem with the form when submit data to mysql database

I receive the error message "Response: 500 Internal Server Error"

no error display all tools for mysql connection not display data and display page without data

I copy all codes, and css and JS to VS project it work fine but not work from WWB I preview the project by use baby web server SE and make seting like in help page

yes my setting is correct, when I connect database from visual studio and my by the same setting make connection

I create the database and 3 tables with the sql code in help and follow the help instruction but when i add the cms card it do not display any data, although I connect it to the database and i fill the table "cms_pages" with data.

but when i use icons in menu and set icon font to Font Awesome 5 solid the icons seen as squares when i publish or preview the page online
that happen when i use manually specify font-face fonts and add fonts i used in design.

I use the grids instated of bookmarks but I have another problem in mobile version

I set the panel menu as fixed menu but when I click any link it move to the bookmark and the panel menu scroll with the page i need it to be fixed and the page only scroll
