The submit button seems to works correct.
However, your form is using 'mailto'. This is not a reliable way to process forms.

See this related FAQ:
"Why shouldn't I use 'mailto' in the form action?"

hello and thank you for your intervention but can you help me find another program that allows me to complete the forms because it was more than a year ago that I still had the same problems with the forms and while I followed to the letter the methods ...

Thank you for your help

500: Internal server error
This error is generated when a script running on the server could not be implemented or permissions are incorrectly assigned for files or directories
Troubleshooting suggestions:
Temporarily disable any rewrite rules by renaming your .htaccess file if it exists.
Ensure that any CGI or Perl scripts have at least .755. permissions.
If trying to run PHP and you get this error, you may have an invalid php.ini in your /cgi-bin, or may be missing your php.dat file in this folder.

and the company always tells me this is not everything is ok on their side I do not know yet who to address this help me to find another software for forms

500: Internal server error
This error is generated when a script running on the server could not be implemented or permissions are incorrectly assigned for files or directories
You can see from the message this has nothing to do with the program/software it is a server related error. As mentioned in the Trouble Shooting suggestions, you yourself will need to ensure all files are uploaded to the correct folders and that all permissions are set correctly.

You may have a syntax error in the code, but to be able to help we need to see the php code only (not the HTML). Please try not to blame the program/software as it is a server fault or it is a error on your behalf (not being rude). Please also note hosting companies always try to blame others when the issue is with them and they simply don't want to help.

Sorry I cannot help to recommend another software to build forms with as you may have issues using it to which we cannot help you with.

Please see the following as a alternative form hosts.
What if my host does not support php mail or php mailer?
viewtopic.php?f=10&t=32967

See the following from Google on 500 Internal server error.
https://www.google.com/search?q=500+Int ... e&ie=UTF-8

Also the extension name of the form is incorrect it should be
http://mysteredelatrinite.com/gourou_vital.php
and not
http://mysteredelatrinite.com/gourou_vital.html

Forms do work if you configure them correctly.

What are your exact settings?
What is the PHP code of the page?
Or where an I download your project?

Hello, I work hard to have a good success but I have this message when I send the message:

404: Page not found
This error is generated when there was no web page with the name you specified at the web site.

Troubleshooting suggestions:

Ensure the page you are linking to exists in the correct folder.

Check your file name for case sensitivity . Index.htm is not the same as index.htm!

Temporarily disable any rewrite rules by renaming your .htaccess file if it exists.

N.B: I try : html and htm I always this same message.

php code:

<?php $mailto = "gourouvital@mysteredelatrinite.com";
$subject = "Feedback form";
$message = "Values submitted from web site form:";
$header = "From: ".$_POST['email'];
foreach ($_POST as $key => $value){ if (!is_array($value))
{ $message .= "\n".$key." : ".$value;

} else

{ foreach ($_POST[$key] as $itemvalue)

{ $message .= "\n".$key." : ".$itemvalue;

}

}
} mail($mailto, $subject, stripslashes($message), $header); ?>

thx you

As previously mentioned to you, when you are using a page with a form it will use php to process it on the server, this means that the page extension should be .php not, .html or .htm but .php!!! Any errors you may receive are generated by the server due to misconfiguration by yourself and not the software. Only you can fix these or consult your host for assistance.

Did you upload all pages/files?
What is the complete PHP code of the page?
Does your web host support PHP mail?
Are you sure it is enabled working properly?

What is the exact problem?
What do I need to do to see the error?
And why didn't you use the built-in form processor script?

I'm sorry, there is no standard calendar extension.
However, you can integrate any third party script with the HTML tools.
