Form causes website error

I have a form on my site..
it worked perfectly in WWB14 but when I upgraded to 15
Form setting
Email address

now shows
from email
From name
Reply to: $mailfrom <--is automatically defaulted

When I didn't receive the FORMS entered info. I noticed that being the only difference so i removed the $mailform.
Uploaded and visited the site (page) and the page does not show up at all it shows error instead of the page.
"This page isn’t working *** url removed *** is currently unable to handle this request.
HTTP ERROR 500"

The site is: ***url removed *** its showing because I replaced the $mailfrom but still not receiving the submissions.
Going to try and revert to a working WWB14 file and upload.

'$mailfrom' shouldd not be removed, this will make the script invalid.
Please restore this value.

Also make sure that you republish all files. Because the upgrade has a lot of updates files.

I thought I did upload everything but i will try again.

The email script should work right out of the gate?
All i did was update to 15, change something that had nothing to do with the FORM and upload. It stopped sending the submission.
We will see.

Yes, the script works, it has been successfuly used by many other users.

If emails are not sent then it's usually a server related issue. See the FAQ.
