Hi,
I'm looking for information, if possible, for a simple data collection extension or code.
If possible:
- Have a dropdown to select person name
- have a date selection
- enter time
- add comment.
If possible have an excel backend but not essential.
Thanks for any suggestions.

This is a very specific requirement, there is no standard extension for this.
You can create the layout of the form in WWB, but you will need a custom script (PHP) to process the data.

I have not learned the WWB form yet.  I still use jotform.
I believe you could build a jotform with a dropdown and then enter the info you specified.
