Select field not transmitting multiple selections

I am hoping this can be answered without uploading the website. I have a select field on the form in which I selected “allow multiple selections”. And, on the published site it allows that. After I hit send I check my email, but it only reports one item when I selected 3. Is there something else that I need to change in the field configuration?

"When you use this then each item name needs to end with [ ] (square brackets) so they look like this item1[ ] item2[ ] and so on."

Followed the link and it helped. However, I decided on the order form to add 5 total select fields and turn off “allow multiple selections”, but when the form is filled out and sent it only reports one field and ignores the other 4 that were filled in. Each field is named select1, 2 ,3 ,4 …etc. what and I missing? Thanks for the help.

I think I fixed it by filling in the title field.
