Form data not captured

My contact form mail is received in the intended email.
1. No submitted data is captured except the ip address.
2. After submission, the webpage goes to a blank page where as it should come back to the blank contact form page

Where am I doing wrong?

Viewing the page source it looks like none of the Form elements/fields are part of it. Please move all the fields on to the form for it to work as intended.

Next problem:
After form submission the form should me reset and blank contact form should be displayed.

I am getting a blank page.
