Password of the server where to give in forms

Dear Pros,
I know I am inexpert, it is so basic thing...
but I haven't used this part of the software for more than four years.

Now I am not able to find

where I can give the password of the server of my www. myname .com web site.

For two days I have been searching everywhere - without results. I remember I did use it but

I am not able to find the password option in 'form' part, so the form is unable to connect the server.

Actually I want to put there a questionnaire with about 15-20 questions and I want to save the answers and the names and the emails addresses in a csv file.

If you have a compact solution (free or not free) or

somebody would build a template for me I would use it with pleasure.

Pls help me. Thanks a lot.

You can use wwb’s built-in PHP Form Processor to do this.
The server’s password etc. is not entered in the form but is specified in the Publish properties when you publish the web page to the server.

If someone has a full template, I would be grateful. I am an amateur.

You need to create a publish profile in the Publish settings first.
