PHP Form Setup Issues

If this has been covered I apologize, I have searched and not been able to find my issue.
I am using GoDaddy with Cpanel
I have used the built in PHP processor to collect data from a form and email, however, the emails are not being sent. Page extension is set to PHP and I followed the directions in the tutorial, I feel like this should be pretty straight forward but something is not right. Any help you can provide would be greatly appreciated.
