Form OK on PC, somewhat on Iphone and not at all on Android

New site under construction using a form in a block. My host has php enabled and the page suffix is php. The preview shows the form at all page sizes and when I view the site on a PC the form appears as designed including at the two breakpoints.

But on Android (Firefox & Google) a blank space appears where the form belongs. On an I-phone SE the form appears but only the 3 of the 5 edit boxes appear. The form is at the bottom of the one-page site,

I do not have an Android device, but it looks correct on my iPhone.
Maybe you will need to clear your browser cache?

Thanks, I had tried that without any change. I then tried other Android phones but the form remained absent. So I deleted that particular block-form and put in a new form style and it works. Appreciate you taking a look though.
