I've been beating this issue for a few days now and cannot find a reason "why". I need to know why so that I can correct the misunderstanding.

The issue is that the menu in the example is inactive. The drop-down menus don't work, and the entire menu seems to be disabled.

Any suggestions to follow through with?

There is an error in the validation settings of 'SQ1'

You need to enter a name for your Mothers' maiden name

should be

You need to enter a name for your Mothers\' maiden name

Otherwise the quote will break the java-script syntax.

The issue is resolved and I don't think I would have found that problem.

I'm astonished that you found it, and so fast.

You can look up errors yourself in the error console of your browser (F12).
If you learn how to use this tool then it may save you a lot of time.
