What do I need to do to fix my contact form?

Here I have a contact form that seems to work but the emails never arrive in my inbox.

I checked through the FAQ and tried to fix what I could but the emails still never arrive. I have the page set to php, i tried changing "email" to "e-mail" editbox, I tried setting a different "from" address. No luck Could this be a server / domain issue with my web host? We are on Hostinger who offers the latest version of PHP, and we SEND emails from this address, so it seems to be email-functional.

Your form seems to be ok.
Did you ask your web host why the message is not sent?
Maybe you will need SMTP authentication?
