How to add items to a form?

Can anyone help me with this? I need a form where I can add items to as I need them.

I don´t know how many items will use the visitor, so I want let him add as many fields he need.

Something like a "+" plus sign so the visitor can add more info to the form.

There is no standard solution for this, this requires a custom script.
