Why don't you have look at some existing sites/businesses and get some ideas/inspiration to build your own?

From previous replies no there is nothing available, hence the suggestions to either modify an already existing one, or take ideas from other sites to make your own.

I agree - nice site, but just a couple of points.

1) Think a about a Go Up.
2) Use a Captcha with Bootstrap real time validation in your form.
3) Link back to your website in the footer.
