I have a problem with the contact form. My hosting operator has blocked the option to send a form where only an email address is provided. This form must have an address, smtp and port. Please help

Sorry this is only a url to your site what has been asked is that you provide a copy of your project this means a copy of the .wbs file. Then upload to your site / server and provide a download link or upload to Mediafire and provide a url to download from.

I assume the contact form you are referring to is the 'kontakt' page?
It seems to be correctly configured.

You will need to enable 'upload files to a folder on the server' in the advanced settings.
Note that the file size may be limited by your web host.
