strange code showing at the top on my index page

Link: Is this being caused by my server?
First time uploading my index page onto the server.

Have used a seperate index only build for a parking page and it displays perfectly.
Error report shows nothing that might lead to this errorI think.

This is the form script, you will need to set the page extension to PHP.
