how do I paste the mysql code into php myadmin

Hi I am looking to setup a database using mysql on my hosting account.
I have created a database on the server.
I have a few pages for a site for a members login and completed the settings in the admin page and generated the code to create the database table on the server.
I have logged into php myadmin and clicked on the SQL tab but I cannot find where the code needs to be copied to to create the table.

Anyone know where I need to be pasting the code to finish the setup?
