Create 2 Promo Codes in one form

I want to make two promo coupons through contact one form - PROMO1 and PROMO2
Which is activated when is
- Service 1 for PROMO1 is selected
- Service 2 for PROMO2 is selected

Can't you just use a combobox with two options?
