Form for multiple choice and quantity.

I have been doing an awkward form for the customer where I use a drop down list. Customer choices one from list and has to choose quantity in the cart. Then he has to come back and choose another option on the list and go through the same drill.
It is a bit annoying for the customer.

I have been hoping for a long time for a form where I could have the options and a quantity beside each option before it sends it to the cart.
Is there anyway to pull that off or an extension that would work?

When you click 'Quick View', you can select an option and quantity.

How is that done with the 2 fields tied together for the buy button?
